When you enter Residential Care there are many things to consider, decisions to make and a new life to adjust to!
We understand the importance of choice and providing a good quality of life by respecting each individual’s independence. We support our residents to make their own decisions, to understand any identified risk and how it can be managed appropriately and within the capacity of the individual. We encourage our residents to live the life they choose and to receive the care and services they require and to live as well as they can within their capacity.
Our Residents Handbook covers all the ‘ins and outs’ of life at Maroba with one of the topics covered being ‘Your Care’.
Clinical Care
Our Clinical team includes the Director of Care, Clinical Care Partner, Registered Nurses and Enrolled Nurses. This team work with residents to assess their health requirements and whenever a change in their health occurs we do timely assessments to meet care provision and support optimal outcomes. We take a multi-disciplinary approach in meeting a residents needs and will involve all key parties to ensure we achieve a residents goals and optimal outcomes.
Advanced Care Planning
Advanced Care Planning is encouraged as it informs resident’s family and everyone involved in their wellbeing what type of treatment, care and service they do or do not want should they become unable to make their own health care decisions and/or are palliating. In short, it ensures residents have control of all of their decisions including those concerning end of life and allows them to maintain dignity at this time.
During the planning process, residents may wish to consider discussing the following with their family, doctor and others who are close to them:
- What is important to them when they are dying?
- Are there specific medical treatments they especially want or do not want? (These could include diagnostic testing, artificial feeding, cardiopulmonary resuscitation, surgical procedures and hospital admission.)
Maroba supports residents right and choice in planning for their future health care needs and It is important that this is done while residents still have the capacity, otherwise family members can be placed in a position of uncertainty when the time comes.
Care Plan
We are committed to ensuring we know and understand our residents, so we can ensure their needs are respected, valued and they feel safe. Comprehensive assessments are undertaken with care plans developed to reflect residents needs, goals, preferences, promote health, well-being and engagement so they can live their best life. Any support they require will be identified and offered as part of this process. We understand that they have the right to make their own decisions about their care and services and to take risks.
Case Conferencing
Case conferencing plays an important role in communication and by involving all key people who are actively involved in residents care we can ensure they receive holistic care. We will work in partnership with residents in assessment and planning to optimise health and well-being in accordance with their needs, goals and preferences. There are a number of ways we hold case conferences such as in person, phone, zoom or FaceTime. The Registered Nurse is available to discuss care requirements with residents and family to ensure that we are providing appropriate care to meet a residents needs.
Palliative Care
Maroba implements the best-practice guidelines of a palliative approach to care for all residents which means as a residents health declines due to the ageing process and or other chronic health conditions deteriorate, the goal of care will change. It will move from a focus on curative measures to identification of pain, physical, psychological, social, spiritual and cultural needs in order to improve quality of life.
When we make a decision on when to begin palliative care we take into account not only a residents diagnosis, but also a residents needs and consult with the resident, family and any others close to the resident whom they have nominated to be involved in their care.
